W-4 Form

This is the form your new employee will fill out to let you know how many allowances he or she is claiming for federal withholding purposes. You will use the information in this document for payroll to determine how much to withhold from the employee’s paycheck each pay period to pay his or her federal taxes.

This is the form your new employee will fill out to let you as the employer know how much to withhold from your employee’s wages to cover his or her state tax liability.

I-9 Form

This form must be completed within 3 days of the new employee’s start date. The purpose of the I-9 Form is to verify the identity of your new employee as well as his or her eligibility to work legally in the United States.

Maryland New Hire Registry Form

This is a mandatory form that must be filled out by employers within 20 days of hiring a new employee. It’s purpose it to expedite the collection of child support from parent(s) who change employment regularly and help to locate non-custodial parents more quickly so that child-support orders can be established.
